We'll miss you, Dr. Cook!

Pediatric Health would like to thank Dr. Jason Cook for his hard work, compassion and the excellent care that he has given to our patients over the past couple of years. In early April Dr. Cook will be relocating to Minnesota to be closer to family. We wish him all the best and know that no matter where he goes, he will always touch the hearts and lives of everyone he meets.

For families who have Dr. Cook listed as your child’s pcp with your insurance carrier, you will need to choose another physician at this time. If you would like to continue to have your child seen here at Pediatric Health Associates, you may choose any of our physicians as each participate in the same health plans.

Jason, thank you for brightening our days! It’s been a pleasure and honor to work with you. We will miss you!

My 18-month old son has been seeing Dr. Cook for his whole life. We really like Dr. Cook and will miss him so much. Dr. Cook helped us receive early intervention services for our son. Thank you for everything Dr. Cook. L. and J.

Dr. Cook is the one that helped diagnose my daughter with Celiac Disease at only one year old. After a very difficult 8 weeks that was a comforting diagnosis. He was really great with her care. Thank you and best of luck! K.& A.
