 ATTENTION ALL KIDS PATIENTS!

The State of Illinois will be moving many patients into a managed care program.   If you are selected, you will be receiving a letter asking you to pick which managed care program you want to be part of. 

Each of these programs will provide you with benefits that may not have been offered to you in the regular All Kids program such as transportation services.

IF YOU DO NOT CHOOSE A PROGRAM, ONE WILL BE CHOSEN BY THE STATE FOR YOU.  

You will also need to choose a primary care provider (PCP) from the managed care plan that you choose. 

IF YOU WOULD LIKE FOR YOUR CHILDREN TO REMAIN PATIENTS AT PEDIATRIC HEALTH ASSOCIATES, IT IS IMPERATIVE YOU CHOOSE A PLAN THAT WE ARE CONTRACTED WITH.   Please call our office at 630.717.2300 to find out which plans we participate in.

You may also call the State of Illinois Client Enrollment Services line at 1-877-912-8880.  Give them the name of the provider you want to have as your PCP and they will be able to provide you with the plans that physician participates in.

REMEMBER, if you do not choose a plan and a provider for our practice, you will no longer be able to be seen here as the state will require you to only be seen at the provider you have chosen.

DO YOU HAVE AN APPOINTMENT IN THE NEXT COUPLE OF WEEKS WITH DR. WALL?

If you are a parent of an existing patient who is seeing Dr. Wall for a well check in the next couple of weeks, you may be asked to fill out a survey.  This survey is a research project by the National Center for Family Professional Partnerships and the American Academy of Pediatrics.  

The survey is completely anonymous & voluntary-- so we hope that you will participate!   There is no id number or name on the survey to know who filled out the form.  You will be given an envelope to put the survey in and seal.   You may hand the survey to the front desk and they will in turn put it in a mail folder that will go directly back to the researchers.

I'm glad you mentioned that you need to make sure you pick a plan before getting any sort of pediatrics done. I can vouch for that. Pediatrics are so important to monitor your child's growth and to make sure he/she is developing correctly.
